Basic Savings Plan

  • Adult 18 and over, only $379
  • Child 17 and under, only $349
  • Periodontal plan, only $599

Coverage

Diagnostic & X-Rays

  • Comprehensive Exam (New patient-initial visit) 100%
  • Periodic Exam (2 per year) 100%
  • Limited Oral Exam (Problem focused-2 per year) 100%
  • Full set of X-rays or Panorex (1 every 5 years) 100%
  • Periapical- First film 100%
  • Periapical Second film 100%
  • Bitewings (One set per year) 100%

Preventative

  • Child Prophylaxis (Cleaning, 2 per year) 100%
  • Adult Prophylaxis (Cleaning, 2 per year) 100%
  • Additional cleanings per year 15%
  • Fluoride 100% (2 times per year)
  • Sealants 15%

Periodontal Plan

  • Scaling and Root Planing 15% (Includes all services under Diagnostic & X-Rays)
  • Periodontal Maintenance (4 per year) 100%
  • Fluoride (2 times per year) 100%

Other Procedures

  • Fillings & Core Build-ups 15% Crowns 15%
  • Veneers 15%
  • Periodontics 15% (Includes 2 Perio Maintenances at no charge)
  • Dentures & Partials 15%
  • Root Canals 15%
  • Implants 15%

Percentages (%) represent dollar amount off individual procedure costs. Anything 100% covered represents no cost to you. Use of CareCredit will only receive 5% discount.

Program Guidelines

  • No credit or refund on any lost or destroyed certificates.
  • No other discounts with this offer.
  • Membership is not for re-sale.
  • Membership valid for one year after redemption.
  • Patient's portion of bill due at time of service.

Program Exclusions & Limitations

This program is a savings plan, not a dental insurance plan, and is secondary to any other dental plan. It cannot be used:

  • In conjunction with another dental plan
  • For services for injuries covered under workman's compensation
  • For treatment, which, in sole opinion of the treating dentist or doctor, lies outside the realm of their capability
  • For referrals to specialists
  • For hospitalizations or hospital charges of any kind
  • For costs of dental care which is covered under automobile medical
  • In conjunction with coupons or cash discounts All products
  • Discounts do not apply to Invisalign or Whitening procedures

This plan is only honored at Emerald Family Dentistry and is NOT an insurance plan that can be used with any other dental office.
